The following paperwork must be submitted ONLINE to the National High School Rodeo Association through your contestant’s Membership Portal https://nhsra.equestevent.com/
- NHSRA Membership Application
- NHSRA Notarized Minor's Release Form (Both Parents/Member must sign and form must be Notarized)
DISTRICT 7 CHECKLIST:
The following paperwork (see LINKS below) must ALSO be submitted ONLINE to the National High School Rodeo Association through your contestant’s Membership Portal
- District 7 Membership Application
- District 7 Medical Release Form (Both Parents/Member must sign and form must be Notarized)
- IHSRA Membership Polices (This has recently changed to combine the forms into 1 document now, it's ok to upload it together as one document with several pages. Upload it into the State Documents on the NHSRA portal.)
- Most Recently Completed Semester/Trimester/Quarter Report Card (Must have the School Stamp and Registrar/Administrator Signature)
- Birth Certificate (Only Required for New members or members Moving from Jr High to High School)
- District-To-District Transfer Form (if applicable)
- Home School Form (if applicable)
